The Wider Practice Team – Care Navigation Sheet
The Care Navigation Sheet has been developed to promote the understanding of the new NHS role of Care Navigator developed for Primary Health Care. Please check with your practice to see if they have this service in place.
The purpose of this sheet is to help patients understand the potential referrals for their treatment when they make a telephone call to their GP practice. Referrals may not always be to a GP; instead, patients may be directed to the most suitable professional. This approach aims to improve access to their care in a more efficient and timely manner.

The Care Navigation sheet promotes an understanding of the range of healthcare professionals and personalised care roles.
