What is it?

The Wellbeing Together programme was designed to support young individuals aged 17.5-25 as they prepared to transition to adult mental health services, with a strong focus on fostering independence.

Our services included:

  • Short-term therapeutic interventions

  • Community engagement

  • Connecting young people to relevant local resources.

Using a social prescribing model, Barnet Wellbeing Together assisted young adults living with mental health challenges in their recovery journey, helping them thrive within their community. Our non-clinical support was tailored for young adults recovering from severe mental health conditions, enabling them to live independently.

Target Audience

We helped young people who were moving from CAMHS to Adult Mental Health Services.

For those who no longer needed adult mental health care, we worked on connecting them to the CB Plus YPT Team, although this was still in progress. Our therapists supported these young adults in adjusting to life in mainstream society.

Where did it run? 

Two specialist practitioners based at Springwell, Barnet Hospital, provided our support as part of the 18-25 Transitions Pathway team, a collaboration between the Integrated VCS and BEH NHS Community Mental Health Teams.

How To Access This Service:

This service was available through CAMHS, BEH (Barnet, Enfield, and Haringey) Mental Health Trust, and the NHS. For young people transitioning out of full-time mental health care and back into everyday life (whether returning to school, university, or other routines) this change could feel overwhelming.

Since October 1st, Barnet Wellbeing Together has moved over to Hestia.
