Last Thursday, we gathered at The Meritage Centre for Hub Connections, a vibrant event chaired by Dr. Clare Stephens. Healthcare professionals, community members, and volunteers came together for a day of mutual learning and open dialogue. The event aimed to bridge the gap between the healthcare system and the community. It offered clear insights into healthcare processes and provided an opportunity to learn from resident experiences.

Breaking Down Barriers in Primary Care

The day began with a compelling presentation by the CB Plus Primary Care Group (PCG). Margaret Singer showcased how the PCG supports our community with practical, person-centred solutions. She introduced the Patient Information Sheet and gave us an exclusive sneak peek at the upcoming Care Navigation Sheet. This new resource will explain the various roles in primary care and simplify the referral process. The session highlighted the importance of transparency and understanding in healthcare.

Engaging and Empowering Through Knowledge

Next, a fun, health-themed quiz energised the room as participants tested their knowledge. The quiz wasn’t just about competition; it reminded us that learning about health can be interactive and enjoyable. The event continued with informative sessions from Dr. Joanne Yong and Tanya Brazza. They discussed vaccines and shared essential tips for community protection. While the pandemic may be behind us, the message was clear: basic hygiene practices—like washing hands and staying home when unwell—are still essential in safeguarding our health.

Moving for Better Health

Physical wellbeing was a key focus of the day. Angelo and Vanessa led two exercise and wellbeing sessions that got everyone on their feet. These sessions demonstrated that movement and activity are vital for a healthy lifestyle. Alongside these energising moments, we learned about the NHS Your Local Health Team campaign. In Conversation with Colin: Building Trust and Awareness

One of the most insightful parts of the event was our conversation with Colin from CB Plus Primary Care Group. With extensive experience at St Andrews and contributions to Central London Community Healthcare NHS Trust, Colin shared his perspective on building long-term relationships with patients and enhancing community engagement. He explained:

“It is the understanding and awareness that makes all the difference.”

Colin discussed the challenges many face in navigating the healthcare system, noting that older individuals, in particular, can feel overwhelmed. He observed, “Working in the surgery, there is a lot of ignorance of where to go. Seniors tend to be totally overwhelmed.” His insights emphasised the need for clearer communication and community-focused initiatives like Hub Connections.

He also highlighted the benefits of long-term relationships in healthcare. “Over a 2 year period, I have built a good relationship with patients,” Colin remarked, stressing that personalised care and continuous engagement foster trust and improve outcomes. Additionally, Colin touched on the public’s hesitancy to engage with the NHS, often stemming from past experiences. His candid reflections serve as a reminder of the ongoing challenges—and the vital role events like Hub Connections play in overcoming them.
