The format can be adapted or translated to meet individual patient needs. In addition to organising their medical history in preparation for a call, the form allows patients to record the outcomes of their consultations.
After the call, it can be challenging to remember all the details of the conversation and the next steps for treatment. This form has been designed to help with this.
